A river should never become the easiest exit route for healthcare waste.
That concern has come into focus after expired medicines and medical supplies were allegedly found dumped near the Mayurbhanj District Headquarters Hospital in Odisha on August 26.
The reported material included antibiotics, masks, syringes and testing kits. The district administration has not yet established the quantity, value or source of the material, or confirmed that it came from the hospital.
Why Water Disposal Is Dangerous
Healthcare waste is not all the same, but a portion of it can be hazardous. The World Health Organization estimates that about 15% of waste generated by healthcare activities is hazardous, including infectious, toxic or otherwise dangerous material. Poor management can expose workers, communities and the environment to biological and chemical hazards.
Pharmaceutical waste creates another concern. WHO has warned that environmental pollution, including contamination involving antibiotics, can contribute to antimicrobial resistance. Its guidance on pharmaceutical waste in healthcare facilities focuses on preventing unsafe disposal and reducing environmental exposure.
India’s Waste Problem Is Measurable
There is evidence that pharmaceutical and antimicrobial residues are already present in parts of India’s wastewater environment.
A study published in Nature Communications analysed 381 sewage samples collected across six Indian states between June and December 2023. Researchers detected 11 antibiotics from seven drug classes and identified 82 antimicrobial-resistance genes associated with 80 mobile genetic elements.
This makes responsible disposal important because antibiotics do not simply become environmentally harmless because they are no longer usable as medicines.
Odisha Has Rules And Facilities
Odisha has a formal regulatory framework for biomedical waste. The State Pollution Control Board says healthcare facilities and common biomedical-waste treatment facilities handling regulated waste require authorisation under the Biomedical Waste Management Rules, 2016. The rules cover the generation, collection, storage, transportation, treatment and disposal of biomedical waste.
The state is also assessing and expanding treatment capacity. Its November 2025 gap analysis covered 6,267 healthcare units across 30 districts, with reported biomedical-waste generation of 21,504 kg per day.
Under the assessment’s assumption of a 20% increase in healthcare units over the next decade, projected generation rises to 25,804 kg per day.
Mayurbhanj is included in the coverage area of Utkal Envirocare’s common biomedical-waste treatment facility at Soro, Balasore, according to that assessment.
Authorities Promise Probe
Mayurbhanj Additional District Magistrate Netrananda Mallick said the administration would inform the District Magistrate and conduct an inquiry if the reported dumping is confirmed. He said action would be taken according to law.
Authorities have not yet identified who discarded the medicines or established whether they came from Mayurbhanj District Headquarters Hospital. The precise quantity, value and source of the expired medicines and medical supplies also remain unverified, according to the report.
Disposal Cannot Be An Afterthought
The Mayurbhanj investigation therefore matters for more than establishing who dumped the material.
If the supplies are ultimately traced to a healthcare facility, authorities will need to establish how they were stored, when they expired, how they were recorded and how their disposal was authorised. If they came from elsewhere, the same chain of accountability needs to be established.
Odisha’s continued efforts to expand treatment infrastructure show that disposal is recognised as part of the healthcare system. The state was still considering new common biomedical-waste facilities and capacity expansions in 2026.
But infrastructure only works if waste enters the system.
The Mayurbhanj allegation should therefore be investigated without prematurely blaming the hospital or assigning an unverified monetary value to the material. At the same time, the broader lesson is straightforward: a water body cannot substitute for a waste-management system.
Medicines may expire. Medical supplies may become unusable. But once healthcare waste is generated, responsible segregation, collection, treatment and disposal are not optional final steps. They are part of protecting the same public health that the healthcare system exists to serve.
